PHYSICAL

A physical exam and vision exam is required for all students entering kindergarten and 7th grade and/or transferring from outside the state. Exams can be completed up to six months prior to entrance into schools or upon transfer into DPS from outside the state. The vision exam may be completed by a health care provider or an optometrist. A complete dental check-up is strongly encouraged at this time also.  Updated immunizations are also required of these students.

Required school physical exams are to be done on or after March 1 of the upcoming school year. Your child's physician will give you the documentation required for a school physical.

If your child will not be receiving immunizations, please return a notarized MEDICAL EXEMPTION FORM.

  • Required Immunizations for Kindergarten-6th grade:

    • 3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday

    • 3 doses of Polio vaccine

    • 3 doses of pediatric Hepatitis B vaccine

    • 2 doses of MMR or MMRV vaccine, given on or after 12 months of age, separated by at least one month

    • 2 doses of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider

  • Required Immunizations for 7th -12th grade:

    • 3 doses of DTaP, DTP, DT, or TD vaccine, one given on or after the 4th birthday

    • 3 doses of Polio vaccine

    • 3 doses of pediatric Hepatitis B vaccine, or 2 doses of adolescent vaccine if student is 11-15 years of age

    • 2 doses of MMR or MMRV vaccine, given on or after 12 months of age, separated by at least one month

    • 2 doses of varicella (chickenpox) or MMRV given on or after 12 months of age. If your child has had varicella (chickenpox), they do not need any varicella vaccines. Written documentation must be provided (including month and year) of varicella disease from the parent/guardian or health care provider.

    • 1 dose of Tdap
