Great news for parents...Thayer County Health Services will be offering sports physicals at their Deshler Clinic on May 19th (12 PM - 4 PM) and 20th (8:30 AM - 10:30 AM) for all INCOMING 8th - 12th graders. Physicals given during the school day on these two dates will have transportation to/from the clinic provided so long as parents/guardians sign up for one of the offered slots AND
- Have all paperwork filled out & sent with your child prior to the appointment.
- Have payment for the physical fee with them at the appointment ($35, separate check needed for each child)...AND ALL MONEY PAID GOES BACK TO OUR SPORTS BOOSTERS!
- Your child is entering grades 8 - 12 this fall (NOTE: 7th graders must have a separate appointment time).
If your child is a legal driver, he/she is encouraged to take an appointment that is outside of the school day and transport himself/herself to the appointment here in town. Preference is given to JH students (this year's 7th/8th graders) on May 19th (PM only) as many HS students will be taking semester finals on this date. On May 20th, students will be dismissed at 11 AM and this is also the same day as State Track. Please keep these things in mind when making your child(ren)'s appointment(s).
If you need to make an appointment for more than one child entering grades 8 - 12 this fall, you will need to fill out the form two separate times. Please note that physicals given during the time frames listed will NOT be submitted to insurance.
If you would like to have a physical completed that is submitted to your insurance, please call the clinic to schedule this for the morning of May 19th in the Thayer County Health Clinic at (402) 768-6041. TCHC is offering up two providers and two nurses to the Deshler clinic on the dates/times requested above.
If you are interested in getting this out of the way for the 22-23 school year prior to this year ending, please complete the form HERE. As time slots are taken, those will be removed from the Google Form, so sign up early to get the time you would like for your child(ren). All paperwork will be sent home with your child and is expected to be returned to the school PRIOR to the appointment, but can also be download here & printed off at home:
